Following an online lecture in which we invited emergency lifeguard Watari Hyodo as a lecturer, we will hold a workshop on actually creating a "patient information list" in a hybrid format, mainly in a face-to-face format. Even if you have never created a list before, you can create one with the help of your instructor. Particularly for first-time participants, the content will be even more useful if you attend both the lecture and the workshop.

If you need help filling out the form, especially if you are not comfortable with using a computer, please come to the Japanese Canadian Museum and Cultural Center instead of participating via Zoom. Your instructor or association staff will help you create a list based on the personal information, past illnesses, and medications you bring.
